Stage 7: Approval/Acceptance

Please review the hairpiece with your stylist before submitting the form.

Approval of the hair replacement system

During the Fitting/Styling appointment, you and your stylist co-signed the Approval/Acceptance form. Signing this document acknowledges that you participated in the final consultation and indicates your:

  • Approval and acceptance of the fitting and styling activities performed by your stylist.
  • Understanding of the care and maintenance guidance provided for your child’s hairpiece.
  • Agreement to consult the care and maintenance guidance, as needed.
  • Agreement to complete the care and maintenance instructions on a daily basis.

Scan and return the co-signed Approval/Acceptance form

Please scan and return the co-signed Approval/Acceptance form to the Wigs For Kids Recipient Department via email (Recipients@wigsforkids.org).

Hairpiece renewal

Wigs For Kids provides one hairpiece, per child, per year. In accordance with our non-profit status, Wigs For Kids is unable to replace a hairpiece that is damaged or in poor condition as a result of improper and/or inconsistent care and maintenance of the hairpiece performed by the child or the child’s family.

Please complete all steps in each stage before moving on to the next stage.
